In order to preserve your rights, you must complete and file a Proof of Claim form (i) at the Claims Office, or (ii) with the Claim Evaluator (whose address is shown below) in order to be eligible for evaluation by the Claim Evaluator and allowance by the Court. The Claims Office will be open on March 3, 2009, March 4, 2009, and March 5, 2009, during the hours of 8:30 a.m. to 5:00 p.m. (Central Standard Time). If you need a Proof of Claim form, and cannot print the form from this site, you can write or call any of the Class Attorney, Plaintiffs' Attorneys, or the Claim Evaluator. Your original Proof of Claim must be physically received no later than 5:00 p.m. on April 18, 2009 ("Bar Date").

FAILURE TO FILE A PROOF OF CLAIM BY THE BAR DATE WILL RESULT IN THE LOSS OF YOUR RIGHT TO PARTICIPATE IN THE SETTLEMENT. THE PROOF OF CLAIM WHICH YOU MAY HAVE FILED IN THE CASE IS NOT SUFFICIENT.
